MyOwnMed provides direct, ongoing connectivity between you and the St. John's Riverside Hospital - RAP project. This application is usable by invitation only. Login credentials will be supplied by your healthcare team at St. John's Riverside Hospital.
If you need to receive reminders for your upcoming appointments, receive a notification to take your medications on time or send a message back to your healthcare team, MyOwnMed makes managing care and chronic conditions as easy as a touch of a screen, providing a simple, personalized experience customized to your own needs.

Medications
Use MyOwnMed to keep track of which medications you are taking, why you are taking them, and receive reminders to make sure you take them on time. Medications are easily added by searching for a drug name or entering the NDC number. You can also add custom medications that may not be found by the drug search.
Vitals
Keeping track of your vitals can be an intimidating task. With MyOwnMed, you will be able to easily add vitals such as blood pressure, glucose, weight, height, and your heart rate. Once you enter vital readings, you can track your historical readings in a graphical or table representation.
Messages
In-application messages and announcements will be sent to you from your provider, and you can also respond to the messages. These may include information such as recent news about your medications and overall tips to live healthier.
Reports
Surveys and reports are available within the application to help you keep track of your well-being, as well as how your medication may be affecting you. A medication adherence report is also available to help you visualize if you are taking your medications as prescribed by your provider.
